INFORMATION AND ACKNOWLEDGMENTS
This application for employment shall be considered open for a time not to exceed thirty (30) days and only for open positions sought to be filled by AVI-SPL at the time of the application. AVI-SPL does not accept applications for “any” or “open” positions or “general” applications.
If hired, I can provide proof of my legal right to work in the United States.

I hereby certify that I understand and agree that an offer of employment with AVI-SPL is contingent upon successful completion of a urinalysis for drug and alcohol use. (AVI-SPL is a drug-free workplace.)
I specifically understand and acknowledge that, depending on the position applied for or the location of employment, AVI-SPL may conduct a background investigation, either prior to or at any time after my employment begins, concerning my character, general reputation, personal characteristics, prior criminal activity, prior driving history, financial history, or similar information, subject to and consistent with all applicable local, state, and federal laws. To this end, I authorize any company, corporation, former employer, credit agency, educational institution, law enforcement agency, or person to provide AVI-SPL any information that they may have regarding me, whether or not it is a matter of public record, and I specifically release any such entity from all liability for providing this information. I understand that, upon written request to AVI-SPL, any investigative information obtained by AVI-SPL will be provided to me within a reasonable time period. I further understand and acknowledge that I will receive appropriate notice and will be presented with a Background Investigation Authorization and Release Form from AVI-SPL in advance of any investigation AVI-SPL may perform, subject to all applicable local, state, and federal notice requirements.
I acknowledge that every statement made in this Application is true and correct to the best of my knowledge and belief and that submitting any false or misleading information or any omission of information pertinent to this Application is subject to discipline, including refusal to hire or termination from employment.
I acknowledge that nothing in this Application is to be construed as an offer of employment for a specific term. If I am hired by AVI-SPL, I acknowledge that I will be employed on an at-will basis unless I am employed under a contract signed by the President of AVI-SPL or his or her authorized designee that expressly and specifically modifies my at-will employment.
I acknowledge that if I am hired by AVI-SPL, I will be expected to abide by any and all policies, practices, and procedures of AVI-SPL, including, but not limited to, all Equal Employment Opportunity policies and procedures.
